Non-emergency medical transportation (NEMT) covers a wide range of trips: a ride to a dialysis appointment three times a week, a one-time trip home after a hospital stay, or a wheelchair-accessible van for a doctor’s visit. The right provider depends on the trip, not just the city. Here’s what to check before you book.

Confirm the vehicle actually fits the need

Not every “wheelchair accessible” vehicle handles every wheelchair. A power chair or a bariatric chair needs a van with the right ramp weight rating and interior clearance. Before booking, tell the provider the exact chair type (manual, power, transport chair, or bariatric) and ask them to confirm their vehicle can handle it. The same applies to stretcher transport: ask whether the vehicle is a true stretcher van with transfer equipment, not just a large wheelchair van.

Ask about licensing and insurance directly

A legitimate NEMT operator should be able to tell you, without hesitation, that they carry commercial auto insurance and are registered to operate in North Carolina. It’s a reasonable question to ask on the phone before you book, and a provider who can’t answer clearly is worth a second look.

Get pricing before the ride, not after

Private-pay NEMT pricing varies by provider, distance, vehicle type, and whether the trip includes wait time or a round trip. Ask for a specific starting price or quote for your exact trip before you confirm. A provider who won’t give you a number until after the ride is a red flag.

Ask what happens if plans change

Appointments move. Ask about the provider’s policy for late cancellations, schedule changes, and what happens if your appointment runs long and you need a later return pickup. Providers that handle recurring trips (like dialysis or physical therapy) should be able to describe how they manage a standing schedule.

Recurring trips deserve a recurring-trip conversation

If you’re booking dialysis, chemotherapy, or physical therapy transportation multiple times a week, ask specifically how the provider handles recurring scheduling: do you book each ride individually, or set up a standing schedule? Is the driver consistent, or does it vary? Consistency matters more for recurring trips than for a one-time ride.
